The Welcome Letter and Policies
sets out the guidelines for visiting CaliCos Haven, Inc. On many occasions we are providing confidential therapy to
clients and, as such, must request that no one visit the farm for any reason without an appointment. It is imperative
that we maintain the confidentiality of our clients in session and hope that you understand this policy and the seriousness
of abiding by this policy. In advance, we thank you.

The Registration and Medical Consent are for
emergency purposes only. In the event an injury does occur on CaliCos Haven property, and the emergency contact cannot
be reached, upon calling 911, staff of CaliCos Haven shall have access to this form to be provided to the EMTs and/or Emergency
Room personnel for knowledge of your child's medical history and consent to life-saving treatment.

The Protective Attire and Equipment Form provides
you and your child with information relative to the nature of horses and proper attire to wear around horses. This form
is for you to review with your child to hopefully provide the information necessary to keep your child safe when dealing
with the horses.

And finally, every equine facility requires the
review of and signature to a Release of Liability and Hold Harmless Agreement. This document indicates that there are
certain inherent risks associated with animals of any sort, especially horses. By signing this form, you agree to accepting
those inherent risks while allowing your child to participate with the horses and agree to hold CaliCos Haven and its
staff, etc., harmless for any damages or injuries which may arise while working with the animals.
